In order to comply with state and federal guidelines for educational purposes, please fill out the forms below.  Upon receipt of all completed forms and registration, you will receive a class meeting ID.

If using a screen reading program, you may need to select "button" to access the forms.

All forms shouldn't take too long to fill out. Forms will be able to be applied for future class registration, if desired.

• photo/video release (parent):  So we can record all sessions. The recordings will stay with Arctic Vision, LLC

• HIPAA

• Release of Information (parent): providing the educator the ability to communicate with Arctic Vision, LLC

• Confidentiality (parents/teachers): parent or teacher observing class (only if they have a student participating)

• FERPA (Parent): to know your rights as a parent of a student
